Fever without Localizing Signs

چکیده

The list of microbes that cause fever in children is extensive. Relative importance of specific agents varies with age, season, and associated symptoms. The focus of this chapter is the febrile child with occult bacterial infection. Table 14-1 shows the most common causes of serious bacterial infection in children younger than 3 months. The division at 1 month is not absolute; considerable overlap exists. Also, certain viruses, notably herpes simplex and enteroviruses, can cause serious infections in neonates, mimicking septicemia. In children 3–35 months of age, most bacterial infections with no apparent focus are caused by Streptococcus pneumoniae (in unimmunized children), Neisseria meningitidis, or Salmonella spp. (the latter often occurring in association with symptoms of gastroenteritis). Haemophilus influenzae type b has become rare and incidence of Streptococcus pneumoniae infection has fallen substantially since universal administration of effective vaccines began. Other common causes of invasive bacterial infections, such as Staphylococcus aureus, are usually associated with identifiable foci of infection.

Effect of Preventive Supplementation with Zinc and Other Micronutrients on Non-Malarial Morbidity in Tanzanian Pre-School Children: A Randomized Trial

BACKGROUND The efficacy of preventive zinc supplementation against diarrhea and respiratory illness may depend on simultaneous supplementation with other micronutrients. We aimed to assess the effect of supplementation with zinc and multiple micronutrients on diarrhea and other causes of non-malarial morbidity. Changing Epidemiology of Serious Bacterial Infections in Febrile Infants without Localizing Signs

OBJECTIVE Historically, management of infants with fever without localizing signs (FWLS) has generated much controversy, with attempts to risk stratify based on several criteria. Advances in medical practice may have altered the epidemiology of serious bacterial infections (SBIs) in this population. We conducted this study to test the hypothesis that the rate of SBIs in this patient population ...

Etiology of Obscure Fever

Obscure fever is not an uncommon problem in Thailand. We studied 25 children with obscure fever admitted to Srinagarind (university) Hospital in Northeast Thailand. The etiology was identified in 52% of the cases: dengue (40%), leptospirosis (8%), and micrococcus septicemia (4%). Two cases with primary dengue infection developed dengue shock syndrome. The case with leptospirosis developed infec...

Indium-111-labeled polyclonal human immunoglobulin: identifying focal infection in patients positive for human immunodeficiency virus.

Pooled human immunoglobulin labeled with indium-111 (111In-HIgG) was used to identify the presence and extent of infection in patients positive for human immunodeficiency virus (HIV), presenting with either symptoms and/or signs of acute chest infection or with pyrexia without localizing signs or symptoms. Fifty-five studies were performed in 51 patients with suspected chest infection or pyrexi...

Protection against Diarrhea Associated with Giardia intestinalis Is Lost with Multi-Nutrient Supplementation: A Study in Tanzanian Children

BACKGROUND Asymptomatic carriage of Giardia intestinalis is highly prevalent among children in developing countries, and evidence regarding its role as a diarrhea-causing agent in these settings is controversial. Impaired linear growth and cognition have been associated with giardiasis, presumably mediated by malabsorption of nutrients. In a prospective cohort study, we aim to compare diarrhea ...
